Transportation Services Qualifications

An individual must:

• Live in Mecklenburg or Iredell County.
• Have a one time phone referral called in by a medical professional (doctor, nurse, social worker, medical secretary, etc.)

• Have a medical appointment between 8 a.m. and 5 p.m., Monday – Friday.
• A client must be ambulatory or have an escort that will escort him/her to each appointment.

An individual must not:

• Have a mental or physical condition that could possibly put a driver at risk.
• Receive full covered Medicaid benefits.
